CURRENT STUDENTS

MEDICAL RECORDS AND CONFIDENTIALITY

Your ABAC medical record is confidential. Information will not be released without your written consent except in cases of legal subpoenas and some communicable diseases, which by law, must be reported to the Department of Public Health. A confidential health record is created for each student upon completion and return of the Certificate of Immunization form to ABAC. Medical records are maintained for 5 years in compliance with Georgia law and are destroyed in a confidential manner.

It is strongly recommended that students keep a copy of their Certificate of Immunization with their important papers since a student's need to prove adequate immunization to employers and other colleges often arises after the student leaves ABAC.

HEALTH CENTER SERVICES AND CHARGES

Health Center visits and consultation with the nurses, nurse practitioners and physicians are free. If the Health Center staff recommends vaccinations, medications or laboratory tests, the cost of these are kept as low as possible and are generally very affordable.

The Health Center is able to offer a variety of immunizations at prices competitive with local health departments. However, since prices sometimes do vary from county to county, students are advised to call their local health department to check on their prices.

The staff of the Health Center diagnose and treat minor, acute illness. Chronic conditions such as high blood pressure and asthma are not treated on an ongoing basis. Students with chronic conditions are encouraged to establish primary care with private health care providers. If the student does not have a local primary care provider, we can suggest a local source for health care. Any costs of care from private community resources are the responsibility of the student.

The Health Center provides women's health care such as Pap tests and breast examinations. The Health Center can diagnose and treat sexually transmitted infections (STIs). Condoms are available for purchase. Commonly used medications are stocked to dispense to patients. A variety of lab tests in the clinic such as tests for Strep throat and urine pregnancy tests are available.

  • Designation of a alternative representative or decision-maker
  • Acceptance or refusal of recommended care and information regarding the medical consequences of refusal

  • Report whether you clearly comprehend the plan of treatment and expectations
  • Express any concerns about plan of care and your ability to follow it

  • Understand the consequences of noncompliance with the treatment plan and accept the responsibility for refusing treatment or failing to follow instructions including follow up visits or referrals
  • Present any significant concerns to the health care provider
